Secondment/Fixed term opportunity for 6 months to improve service delivery.

* Secondment Opportunity – Existing permanent employees must first discuss this opportunity with their substantive line manager and complete the secondment request form prior to interview. We currently have an exciting opportunity to join the Discharge Team on a fixed term basis for six months. Working as a Discharge Co-ordinator you will as part of a wider team manage the daily process of pro-active Discharge Planning within Forth Valley Royal Hospital to support the safe and timely transfer / discharge of patients from the Acute Hospital, while implementing the DWD ( Discharge without Delay ) principles and a Home First ethos.

We are seeking a highly motivated and experienced individual who will embrace the challenge of this dynamic role. The post holder will possess an understanding of the Discharge Process and be able to engage with patients, carers, colleagues in hospital and within Health and Social Care to ensure safe and robust discharge plans are formulated.

All applicants should possess the ability to prioritise fluctuating workloads, remain focused, work across professional boundaries and be flexible in an extremely busy environment. You must be able to demonstrate good organisational skills, excellent communication skills, problem solving ability and a commitment to continuing professional development.

This post is open to 1st Level Registered Nurses and AHP registered with HCPC

The service covers 7 days a week 8am-4pm, so weekend working on a rota basis is part of the role.
