Client Care Manager – Join an Award-Winning Home Care Provider
Location: Conwy County and covering parts of Denbighshire
Salary: £26,845 - £30,485 per year + additional on-call payments
Hours: Monday to Friday, 9 am – 5 pm (on-call duties required one week in three/four)

In this role, you will assess, review, and coordinate care services to ensure individuals receive the best possible support in their own homes.

Your Key Responsibilities:
Manage and oversee the care needs of individuals, ensuring high-quality and consistent support.
Conduct assessments and regular reviews to identify changing care requirements and adjust care plans.
Build strong relationships with individuals and their families, maintaining open communication.
Liaise with health professionals such as Social Services, GPs, and District Nurses to ensure seamless care.
Support and develop care staff, ensuring they meet high standards of service.
What We're Looking For:
At least 3 years' experience in care (management experience is preferred but not essential).
Level 3 Diploma in Health & Social Care (or currently enrolled and on track to complete it soon).
Strong organisational skills – able to work independently and stay calm under pressure.
Excellent communication and customer service skills, both verbal and written.
IT literacy, with experience using care management systems.
A self-motivated and resourceful approach – able to take initiative and problem-solve effectively.
