Hospital Pharmacist – Band 6/7/8a – South Yorkshire – ASAP start

1. Job Title: Hospital Pharmacist
2. Banding: Band 6/7/8a
3. Location: South Yorkshire
4. Duration: 6 Months
5. Schedule: Monday – Friday | 9AM – 5:30PM
6. Start Date: July
7. Rate of pay: £22 - £37 per hour.
8. WARDS BASED EXPERIENCE IS ESSENTIAL (Candidates who have completed their pre reg in the hospital sector will be considered)
Requirements and qualifications required for this role:
9. Participate in ward rounds to administer patient drug histories and liaise with patient families, community pharmacists and GPs.
10. Providing clinical expertise and being an educational resource for colleagues in relation to medicines e.g. doses, side-effects, alternatives etc whilst helping to address the public health and social needs of individual patients.
11. Manage the process of medicines reconciliation on admittance or following discharge from hospital and liaise with patients and community pharmacists to ensure patients have the medicines that they need.
12. Inpatient dispensary cover required/clinical screening.
