Program Details
The SCC program is a longitudinal, integrated care delivery program that coordinates clinical care for members in a facility setting. The APC within SCC provides care to our highest-risk health plan members as part of an interdisciplinary team.

Primary Responsibilities
1. Manage members through acute medical situations or exacerbations of chronic conditions, including testing, diagnostics, and interventions.
2. Ensure accurate and complete documentation, including ICD 10 conditions.
3. Communicate and collaborate with the interdisciplinary care team.
4. Conduct advanced illness and advanced care planning conversations.
5. Provide counseling and education to patients and caregivers.
Qualifications
Required
* Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program.
* Active and unrestricted license in New Hampshire or ability to obtain by start date.
* Active Nurse Practitioner certification through a national board, such as the American Academy of Nurse Practitioners or the American Nurses Credentialing Center, with certification in one of the following: Family, Adult, Gerontology, or Adult-Gerontology Acute Care Nurse Practitioner.
* Current DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations.
* Ability to gain a collaborative practice agreement if applicable in your state.
* Driver’s license and access to reliable transportation.
Preferred
* Over 1 year of clinical practice experience, preferably in long-term care.
* Physical ability to perform job requirements, including moving equipment and navigating various dwelling conditions.
* Experience caring for patients with complex behavioral, social, or functional needs.
* Knowledge of Geriatrics and Chronic Illness.
* Understanding of Advanced Illness and end-of-life discussions.
* Proficient in computer skills, including electronic medical records documentation.
Additional Notes
Employees must be in active regular status and remain in their role for at least 12 months. To receive full student loan repayment benefits, employees must stay in an Advanced Practice Clinician or Physician role within Home and Community for 36 months.
Compensation and Benefits
The salary range is $89,800 to $176,700 annually, based on full-time employment and factors such as location, education, and experience. Benefits include comprehensive packages, incentive programs, stock purchase options, and 401k contributions.
