Job overview: We are seeking mental health professionals from a registered core profession (RMN, OT, Social worker) to join us for an exciting opportunity to be part of the development of the Primary Care Mental Health Team within the Leeds Mental Wellbeing Service, in partnership with statutory and third sector services. This role offers hybrid working, involving work within GP surgeries, community mental health bases, and home working within a locality hub.
The team adopts a transdiagnostic approach to mental health care, focusing on helpful conversations and biopsychosocial assessments to identify and formulate patient difficulties. We work with patients to explore their experiences, goals, and support needs, including referrals, psychoeducation, and practical coping strategies.
We value diverse skills and specialisms. The successful candidate will work autonomously and as part of a team, receiving training, support, and clinical supervision.
Main duties of the job:
* Provide mental health and risk assessments, brief interventions, and support to prevent deterioration requiring secondary care referral.
* Offer triage, assessment, and decision support to primary care colleagues to ensure appropriate pathway access.
* Contribute to Leeds' mental health framework development under the Community Mental Health Transformation Project.

* Maintain effective communication and participate in meetings.
* Engage positively with service users and carers.
* Develop networks with agencies serving those with complex mental health needs.
* Report to the Band 6 Locality Primary Care Mental Health Specialist and Locality Manager.
* Adapt communication to barriers presented by service users.
* Qualifications: Recognised professional qualification and registration (e.g., RMN, RN (Mental Health), OT, Social Worker).
* Experience in partnership working, specialist mental health, and risk assessment.
* Understanding of mental health and social care agendas.
* Skills in clinical communication, record-keeping, and social determinants of mental health.
