The Calne, Corsham, and Box Community Team is excited to welcome a Community Healthcare Support Worker to our dedicated and friendly team.
We’re offering a fantastic opportunity for a Band 3 Healthcare Support Worker—or someone enthusiastic and willing to train towards Band 3—to work 30 hours per week in a rewarding role that helps shape and develop vital community health services.

Main Responsibilities
1. Implement treatment by following care plan.
2. Keep the patient at the centre of his or her care, ensuring patient and carer participation in decision-making.
3. Undertake the daily care and support of delegated patients.
4. Deliver treatment/care within a community setting as specified in a written care plan, under the guidance of a registered practitioner.
5. Avoid hospital admission and reduce inpatient length of stay to ensure appropriate care in the most suitable setting.
6. Responsibility for the day-to-day care of patients, including monitoring.
7. Support the taking of medication under the authorization of a qualified practitioner.
8. Have good knowledge of infection control to reduce the risk of healthcare-acquired infections.
9. Enable patients to reach their optimum level of independence.
10. Support terminally ill patients and their carers/relatives.
11. Communicate with individuals about routine and daily activities, overcoming difficulties if needed.
12. Ensure the needs and wishes of the individual are documented.
13. Work within a multi-professional team.
14. Undertake specific tasks as delegated.
15. Assist registered and junior staff, referring to registered staff when necessary.
16. Practice the principles of individualised patient care according to policies, procedures, and guidelines.
17. Obtain consent for any treatment undertaken.
The Ideal Candidate
1. Good general education.
2. NVQ Level 3 in health care – or willingness to achieve.
3. Experience of working under distanced supervision.
4. Experience supporting other workers.
5. Ability to work as part of a team.
6. Delivering patient-focused care.
7. Ability to recognize own limitations.
8. Typing/data input skills.
9. Physical skills for manual handling of patients and using lifting aids.
10. Undertake specific nursing/therapy skills.
11. Current full driving license and use of a car during working hours.
12. Willing to work in other areas of HCRG Care Group as required.
13. Driven, self-motivated, good time management.
14. Food Hygiene certificate or willingness to undertake.
