This is a part time vacancy for 27 hours per week.
The DischargeLounge welcomes applications for experienced Band 5 Staff Nurses to come andjoin our team. The Discharge Lounge isan integral part of patient flow bringing patients awaiting discharge down tothe lounge whilst awaiting medication or transport freeing up an acute bedearlier in the day. The job is fast paced, applicants will need excellent timemanagement and prioritising skills to co ordinate safe and timely dischargesfrom the Lounge. The ideal candidateswill be able to work under pressure, have excellent communication skills and acan do attitude. They will work with themulti discipliniary team including transfer of care hub, site manager, socialworkers, rapid response, recovery at home, pharmacy. Prioritising transport for safe discharge andcoordinating packages of care to ensure patient safety. Applicants should befamiliar with red to green and the home first approach. This post ispredominantly for Sunderland Discharge Lounge however staff may be expected tocover at South Tyneside Discharge Lounge.
Main duties of the job
You will beworking closely with site managers and transfer of care hub to pull planneddischarges to the Discharge Lounge to ensure beds are free earlier in theday. You will develop a rapport with theward staff and be available for advice to staff.
The relevanttransport will be arranged, ensuring relevant assessments have been completedprior to discharge and all access issues are resolved. Transport is to be prioritised and coordinated with external agencies to ensure safety of patients.
Ensuring clearconcise documentation at all times.
The DischargeLounge is open 8-8 Monday to Saturday and 8-6 Sunday.

Job responsibilities

* To managethe Discharge Lounge for a sphere of duty in the absence of B6 Discharge LoungeDeputy Manager
* To workwithin the Integrated Discharge Team (IDT ) under the supervise of the B6Senior Nurse using the Discharge 2 Assess / Home First Model as soon as it isclinical safe to do so. This will involve daily visits to adult wards.
* To beresponsible and accountable for the delivery of safe, effective and compassionatenursing care for patients designated to their care.
* To alwayswork to the NMC Code.
* Tosupervise non-registered staff as stipulated by the NMC under the requirementsof delegation.
* Provideeffective leadership and assist with the management and organisation of theward/department in the absence of the Ward Manager or Junior Sister / ChargeNurse.
* To complywith the trusts vision, values and behavioural compact.
Person Specification
Experience
* Experience of working in an acute hospital or community healthcare setting.
* Recent experience of working as a registered nurse.
Skills and Knowledge
* Ability to provide and receive complex, sensitive or contentious information.
* Able to work without supervision.
* Competent in the use of IT such as Microsoft Office.
* Evidence of CPD.
* Knowledge of national health and social care policy.
* Knowledge of evidence based guidelines e.g. NICE guidelines, NPSA alerts, current nursing legislation.
* Venepuncture and cannulation competency.
Physical Skills
* Ability to assist patients to move as per Trust Moving and Handling Policy.
Qualifications
* Current NMC Registration (Level 1 adult nurse) or evidence of qualification if newly qualified.
* Diploma in health related study or equivalent appropriate qualification.
* Degree in nursing studies or healthcare related subject (UK equivalent).
* Mentorship certificate/ Practice Assessor training or demonstrable equivalent experience.
