NorthTandridge Primary Care Network (PCN) have an exciting opportunity to expand its Pharmacist Workforce,including 8 Clinical Pharmacists and 2 Pharmacy Technicians, and build on theexisting team as they continue to recognise the value the additional rolesbring to our practices and patients. Our aim is to provide exemplarypatient care and find innovative solutions in general practice to deliver thebest care we can for our patients. You will be joining an enthusiastic team ofclinicians and administrators. The role will contribute to improving thequality of care of our patients.
OurPCN comprises of 5 Practices covering the North Tandridgearea. The Practices (Whyteleafe Surgery, Chaldon Road Surgery, Warlingham GreenMedical Practice, Elizabeth House Medical Practice and Caterham Valley MedicalPractice) have a combined population of 36,000 patients. There is a widesocio-economic and demographic served within this area which will provide aninteresting varied case mix.
Withinour PCN, we have a number of innovative services that we haveimplemented to identify new approaches in delivering care and improve thehealth and wellbeing of our population including an Anticipatory Care Hub,Children and Young Persons Wellbeing Service, GP Integrated Mental HealthService, BP@Home and Enhanced Access.
Main duties of the job
Thepost holder will work with the patients of Caterham Valley Medical Practice andwill be assigned a lead clinical and operational contact for each contract. CaterhamValley Medical Practice is an established training practice. Our current listsize is 10,295 but is growing fast so you will work as part of a busymulti-skilled clinical team. The ambition is to better utilise the role of pharmacy within primarycare to pro-actively help patients stay safe and well and out of hospital aswell as helping to reduce the demands on general practice.
The post holder is a Pharmacy technician, who acts within theirprofessional boundaries, supporting pharmacists in general practice to ensureeffective and efficient use of medicines. In this role they will be supervisedby a pharmacist who will develop, manage and mentor them.
The Pharmacy technicians core role responsibilities will coverclinical, and technical and administrative categories.

Job responsibilities
Clinical:
1. Undertakepatient facing and patient supporting roles to ensure effective medicines use,through shared decision-making conversations with patients
2. Carryout medicines optimisation tasks including effective medicine administration( checking inhaler technique), supporting medication reviews and medicinesreconciliation. Where required, utilise consultation skills to work inpartnership with patients to ensure they use their medicines effectively
3. Asdetermined by the PCN, support medication reviews and medicines reconciliationfor new care home patients and synchronising medicines for patient transfersbetween care settings, linking with local community pharmacies
4. Supportthe Clinical Pharmacist in Structure Medication Reviews (SMR) organisenecessary monitoring tests prior to SMR
5. Provideexpertise to address both the public health and social care needs of patients,including lifestyle advice, service information, and help in tackling localhealth inequalities
6. Manageshared care protocols and liaise with Clinical Pharmacists for more complexpatients
7. Supportinitiatives for antimicrobial stewardship to reduce inappropriate antibioticprescribing locally
Technical andAdministrative:
8. Supportthe PCN multi-disciplinary team to ensure efficient medicines optimisationprocesses are being followed
9. Supportmedications reconciliation for patients when they are discharged from hospital
10. Implementefficient ordering and return processes and reducing medication wastage
11. Providetraining and support on the legal, safe and secure handling of medicines,including the implementation of the Electronic Prescription Service (EPS)
12. Promotionof Electronic Repeat Dispensing (eRD) and online ordering
13. Developrelationships with other pharmacy professionals and members of themultidisciplinary team to support integration across health and social careincluding primary care, community pharmacy, secondary care and mental health
14. Supportpractice reception teams in streaming general prescription requests, so as toallow GPs and clinical pharmacists to review the more clinically complexrequests
15. Supportthe implementation of national prescribing policies and guidance within GPpractices, care homes and other primary care settings
16. Supportthe PCN to deliver on QIPP agenda, QOF and locally commissioned enhancedservices
17. Supportthe PCN in reviewing and developing practice policies for CQC requirements
