We are looking for a nurse with a genuine interest in continuing to develop skills in this field. We have a good track record of supporting newly qualified nurses, return to practice nurses, and nurses transitioning into the community from other settings including acute care. You must be able to work flexibly according to service needs and have excellent organisational, time management, and communication skills.
Responsibilities include:
1. Providing skilled nursing care based on best practice and clinical evidence, including chronic disease management, end of life care, continence promotion, and wound management. Specific skills include phlebotomy, medication administration via syringe drivers and intravenous therapy, PEG management, urinary catheterisation, and tracheostomy suctioning.
2. Acting as a Named Nurse under the supervision of the DN Team Lead, responsible for holistic health needs assessment, care planning, evaluation, and ensuring continuity of care following referrals from acute care, rehabilitation units, GPs, and other agencies.
3. Conducting comprehensive assessments of patients' physical, psychological, and social needs using the Single Assessment Process, including those of carers. Developing individualised, person-centred care plans that promote self-care and aim to prevent unnecessary hospital admissions.
