Job overview Would you like to work in a fast paced environment with every speciality patient needing care? We are looking to recruit an experienced and motivated individual to the role of Junior Sister/Charge Nurse for Charnley Ward - a short stay ward We are looking for experienced nurses to join our team. You will have strong communication and leadership skills with an eagerness to advance acute nursing practice. You will support the unit manager in this busy environment that receives patients from the Adult Assessment unit and Emergency department. You will be responsible for being the nurse in charge of the shift monitoring patient flow through the unit. The ideal candidates will be dynamic and forward thinking with a progressive approach to enhancing patient care and safety within urgent, emergency and acute care services. In addition to excellent clinical skills a good working knowledge of care pathways and the patient journey is required, as is priority focus on communication, safe discharge planning and safe patient transfer. Rotation to the Adult Assessment Area is also part of this role. This area operates a 24 hour or less length of stay and is working towards receiving referred to speciality patients direct from the ED. Main duties of the job Work according to the NMC Code of Professional Conduct and relevant professional guidelines to wokr as a named nurse (or key worker) for a defined group of patients and take responsibility for: Protect and supporting the health of individual patients and clients Assess care and health education needs Develop, implement and evaluate programmes of care including discharge planning for each patient To be designated to take charge of the ward/department regularly in the absence of the person holding continuing responsibility To work in accordance with the Princess Alexandra Hospitals Nursing and Midwifery Strategy and Core Standards To maintain effective communication To provide a high standard of holistic patient-centered care To act as a facilitator/mentor/preceptor to student nurses and others and participate in clinical supervision programmes as appropriate The Princess Alexandra Hospital NHS Trust (PAHT) is proud to care for the communities of West Essex, delivering safe, compassionate, and high-quality services. Detailed job description and main responsibilities Provide and maintain effective clinical assessment to ensure that patient care is planned and evaluated. Deliver effective nursing care to patients/clients admitted to the ward/unit. Seek supervision to ensure that personal clinical practice is continuously developing. Participate in appropriate training to ensure safe competent clinical practice. Ensure that all documentation is comprehensively completed in accordance with the NMC and Trust guidance. Involvement in monitoring and auditing patient care. Act at all times in accordance with the NMC Code of Professional Conduct. Develop supervisory skills that support the continuous development of team members. Participate in the development of operational processes that will ensure timely and appropriate admission and discharge of patients. Work towards the development of skills that will contribute to an ethos of nurse led services. Participate in the Trust PRDP process to ensure that you receive and give constructive feedback to enhance personal skill set and the promotion of professional development of nursing within the Trust. Act as supervisor/mentor to junior staff members and student nurses
