Durham’s Family Group Conference Service is offering an exciting opportunity to join our dedicated team as we deliver an innovative Lifelong Links programme—funded by the Department for Education until March 2025. This unique role focuses on supporting young people in our care as they approach adulthood, helping them to reconnect with the people who matter most and preparing them for a confident transition into independent living. ⭐ What You’ll Do As a Family Group Conference and Lifelong Links Coordinator, you will: Work directly with young people to explore their family and support networks. Lead on organising and facilitating Lifelong Links Family Group Conferences. Bring families, friends and professionals together to create meaningful, future-focused support plans. Provide mediation and restorative interventions to rebuild positive relationships. Support the wider Supporting Solutions Service by contributing to crisis response and helping prevent placement breakdowns. Work collaboratively with partner agencies to ensure the best outcomes for young people approaching independence.⭐ About the Role This is a varied, rewarding position that combines direct work with young people, family engagement, multi-agency collaboration and crisis intervention. You’ll need to be confident working flexibly, managing complex situations and supporting young people with empathy and resilience. Travel is essential, so a car, valid UK driving licence and business insurance are required. The role is primarily Monday–Friday, with some evening and weekend work. This post requires an Enhanced DBS.⭐ About You We’re looking for someone who: Has significant, recent experience of delivering Family Group Conferences. Is experienced in direct work with young people and their families. Brings a caring, non-judgemental and empowering approach to every interaction. Is resilient, flexible, creative and passionate about helping young people thrive.
