Community and Hospital Support Worker - Fixed Term
We’re looking for someone to work on our Hospital Discharge team. You will collaborate with integrated discharge hub staff to support customers to be discharged from hospital and those who need support to stay safe and well in their own homes in the community.
We will reimburse mileage and parking expenses for any travel undertaken as part of your role.
Responsibilities
* Complete tasks to help customers get home safely from hospital such as doing property inspections, collecting belongings, dropping off food parcels, or arranging quotes for deep cleans.
* Conduct ad‑hoc or regular visits to community members identified as needing further support, checking that homes are warm and safe and that medication, food and other essentials are available.
* Visit wards, engage with customers and source volunteering services to support the customers’ wants and needs.
* Collaborate with other hospital teams to ensure a timely discharge.
* Perform a physically active role: walk approximately 5–6 miles per day while delivering small food parcels.
Qualifications
* Excellent communication skills.
* Openness to working with vulnerable and/or older people.
* Understanding of what constitutes excellent customer service.
* Ability to work under pressure and manage high caseloads.
* Excellent ICT and administrative skills.
* Skill in identifying issues to prevent delays in discharge.
* Hold a full UK driving licence and own vehicle, including business insurance.
* Enhanced DBS required.
