This is a full-time, permanent role with hybrid working and a salary of £37,526 - £41,613 per annum.

The Role:
We are looking for a passionate and community-focused Community Prevention Officer to join ourSystems, Performance and Communities team. Reporting to the Community Prevention Manager, you will play a key role in connecting residents with local resources to enhance their independence and wellbeing.
Your role will involve:
* Acting as a central point of contact for referrals from health and social care.
* Developing personalised community support plans for residents.
* Collaborating with local groups and services to match residents with appropriate support.
* Supporting community capacity building and integration across sectors.
* Monitoring outcomes and contributing to service improvement.
What we are looking for:
* Experience working directly with communities and in partnership with stakeholders.
* Knowledge of local community resources and the voluntary sector.
* Strong communication and interpersonal skills.
* Excellent organisational and time management abilities.
* Confidence using IT systems and managing referrals.
