Overview
Care Navigator (MH Nurse or OT) East of England IBPS – Cambridgeshire and Peterborough NHS Foundation Trust (CPFT).
We are looking for a Senior Mental Health Nurse or Occupational Therapist to be part of the new Infected Blood Psychological Service (IBPS) supporting those who were infected or affected by contaminated blood products. The role is based within CPFT’s Psychological Medicine Service at Addenbrooke’s Hospital in Cambridge and hosted by the Addenbrooke’s Haemophilia Centre.
CPFT provides Psychological Medicine Services within acute hospitals in the county, including Addenbrooke’s Hospital, Royal Papworth Hospital, Peterborough City Hospital, Stamford Hospital and Hinchingbrooke Hospital. The Cambridge Psychological Medicine Service has an established track record and an excellent rating by the Psychiatric Liaison Accreditation Service.
Responsibilities
* Be the first point of contact for people accessing the service and help ensure a positive experience.
* Complete biopsychosocial assessments and interventions; create care plans; assess risk and signpost to other services and agencies as appropriate.
* Lead outreach programmes, establish mechanisms for peer support, and contribute to the development of the new service.
* Work closely with IBPS Clinical Psychologists and the national network of Care Navigators.
Location and Collaboration
The role is based at Addenbrooke’s Hospital, Cambridge, and collaborates with CPFT’s clinical teams across multiple NHS services in the region.
Requirements
* Qualified as a Mental Health Nurse or Occupational Therapist with senior/clinical experience relevant to psychological medicine and infection-related psychosocial support.
* Experience in biopsychosocial assessment, risk assessment, care planning and multidisciplinary collaboration preferred.
