Job description
As the Practice Manager, you will be responsible for overseeing the day-to-day operations of the team, providing supervision to care professionals, managing cases, and supporting the Team Leader in achieving the team's objectives.
Responsibilities include, but are not limited to:
* Providing regular supervision and support to care professionals.
* Overseeing the care management process ensuring that each individual receives tailored and effective support plans.
* Monitoring and evaluating the progress of young people in transition.
* Working closely with the Team Leader to implement team strategies.
* Collaborating with other departments to ensure seamless coordination of services and resources.
* Implementing and maintaining quality assurance measures.
* Regularly reviewing and updating polices and procedures to reflect best practises and compliance with regulations.
Requirements:
* Minimum of 1 year of experience in a social work or care management role, with at least 6 months in a supervisory capacity.
* Ability to manage competing priorities and work effectively in a fast-paced environment.
* Strong knowledge of leaving care services, legislation, and best practices.
* Excellent leadership, communication, and interpersonal skills.
* Commitment to promoting the well-being and independence of young individuals leaving care.
