About the role:
1. Deliver substance use assessments, including risk assessments, needs analysis and treatment planning for residents within the HRS setting.
2. Provide brief interventions, harm-reduction advice, overdose prevention, safer-use guidance and relapse-prevention support.
3. Facilitate engagement with structured treatment, including prescribing services, psychosocial interventions and external health partners.
4. Ensure all interventions are trauma-informed, person-centred, and aligned with best practice and organisational standards.
5. Work on-site to increase accessibility and visibility of treatment support, developing trusted relationships with residents.
6. Proactively engage individuals who may be disengaged, ambivalent or chaotic, using assertive outreach techniques within the building.
7. Liaise closely with HRS staff, ensuring joined-up approaches to risk management, safeguarding, welfare concerns and crisis planning.
8. Attend relevant multi-agency meetings, contributing substance-use expertise to support shared planning for residents.
9. Contribute to residents’ risk management plans specifically relating to substance use and overdose.
About you:
10. Experience of working in the health and social care sector with vulnerable people
11. In-depth knowledge and experience of working with the complexities
12. Understanding of people who have multiple unmet need
13. Knowledge and experience of working in line with NICE guidelines Alcohol Pathways
14. Understanding the importance of multi-agency working within a strength based, asset building approach and maintaining professional boundaries
15. Experience of facilitating multi-agency meetings
16. Experience of and a commitment to, working positively in partnership with a range of statutory and voluntary agencies.
17. Good administrative, recording and reporting skills.
18. Experience and knowledge of safeguarding procedures applicable to children and vulnerable adults
19. Experience of case management.
