Stockport Foundation Trust is seeking a dynamic, innovative Advanced Practitioner with extensive experience to join their team. This is a lead role as a Consultant ACP, aimed at driving the Divisional agenda for advanced practice.
The role is structured around four core functions:
1. A professional leadership and consultancy function
2. An education, training, and development function
3. A research, evaluation, and service development function
4. Developing, implementing, and reviewing advanced practice to ensure high-quality, equitable service delivery
The post involves leading, developing, and implementing innovative care models for patients across community settings, including urgent treatment centres. The post holder will utilize clinical examination, assessment, and planning skills, providing expert advice to professionals, patients, and carers, and contributing to strategic developments to enhance professional standards.
Main duties include:
* Leading and developing innovative care models in the community
* Utilizing clinical skills for patient assessment and care planning
* Providing expert advice and contributing to strategic and operational developments
* Developing strategies to modernize the ACP team and ensure high-quality, patient-centered services
* Delivering clinical practice, research, innovation, teaching, and training (65% clinical, 35% leadership and development)

Job responsibilities:
* Acting as a clinical expert, ensuring patient-centered care and health promotion
* Managing complex cases as an Independent Prescriber
* Developing clinical competencies and evidence-based care models
* Providing clinical advice and training to staff
Management and Leadership:
* Leading professional development and practice change
* Maintaining visibility and contributing to organizational goals
* Supporting and supervising senior staff
* Representing the Trust regionally and nationally
* Engaging in clinical audits and applying research to practice
Communication and working relationships:
* Developing best practice within the service
* Leading and developing advanced practitioner skills
* Providing expert advice and support across community services
* Ensuring accurate documentation and record-keeping
The role requires an MSc in Advanced Clinical Practice, evidence of practice development, leadership experience, and at least three years as a Senior ACP. It is subject to an enhanced DBS check, with costs deducted from salary.
Salary range: £64,455 to £74,896 per annum, pro-rata.
