We are looking for an enthusiastic and friendly chronic disease practice nurse to join our growing team. You must have recent relevant experience as a practice nurse and be competent in usual practice nurse duties. This is on a bank contract, to meet the needs of the practice, on an adhoc basis. Flexible as to days and hours.
Main duties of the job
Practice nurse duties to include;
Diabetes Reviews - to include review of waiting lists, patient reviews by telephone and face to face appointments.
COPD Reviews - in line with QOF guidelines. Experience with Asthma reviews also desirable.
General Practice Nurse duties on an adhoc basis may also be required, such as phlebotomy, child immunisations, wound care, cervical screening.

Addiitional nursing roles required on adhoc basis:
Cytology
ECGs
Routine immunisations/Childhood immunisations
Chaperoning and assisting patients where appropriate who are being examined by another clinician
Requesting pathology tests, for example urine culture, swabs
Following agreed clinical protocols with referral to GPs as appropriate
Maintaining and cleaning equipment
Maintain general tidiness and cleanliness of nurses and treatment rooms
Participation in administrative systems in the Practice
At all times there will be a need to maintain accurate records
* A duty to advise senior nurses of potential problems or errors within the range of assigned tasks
Any other delegated duties appropriate to the post

Qualifications
* Registered Nurse - adult
* Chronic disease management - Diabetes, Asthma, COPD
* Cervical smears
* Baby immunisations
* Adult injections and immunisations
* Travel health experience
* 24 hour BP monitoring
