As an Advanced Practitioner, you will lead on peer development and practice, supports and supervise staff to develop their skills and knowledge in line with related legislation including The Care Act and Hertfordshire's Connected Lives principles to contribute to the ongoing development of the service. Advanced Practitioners are responsible for delivering high quality statutory social care including recognising safeguarding, delivering assessment, supervision and care management functions as well as developing innovative, creative community solutions. Advanced Practitioners are pivotal in developing links with the local community through undertaking multi-agency work with partner organisations, providers, professionals, individuals, families, and carers. There is responsibility for ensuring that all statutory health and social care responsibilities are met and delivered to a high standard including safeguarding, assessment, supervision, and care management functions.
