Overview
This role will be part of the Transition Pathway within the CYPS Mindworks Community Hub. The Transition Pathway provides support for 17- and 18-year-olds as they transition out of Childrens EWMH Services. The transition pathway is for those who need additional support but the appropriate service to meet their needs is unclear. The Transition Support Worker will case manage and co-ordinate support for young people aged 17-18 as they transition, acting as a reliable and consistent advocate over a period of 6 months. They will develop a Transition Care Plan with the young person to identify needs and ensure the right help and support is provided during the transition period and beyond. They will accompany the young person to appointments, assessments and therapy, educate and reflect on differences in culture and practice between services, and help signpost and/or refer to emotionally beneficial services. The successful candidate will attend regular MDT meetings and liaise with a range of professionals, agencies and other parties involved in the young person’s care.
Responsibilities
* Support young people through the transition out of Childrens EWMH Services and into appropriate mental health or community services as they reach 18 years of age.
* Be approachable, friendly and able to engage and build relationships with young people with complex mental health needs.
* Think creatively about how to engage with a young person.
* Be a consistent and reliable advocate/voice for a young person during the transition period.
* Accompany a young person to appointments, assessments, meetings, and therapy as required.
* Develop an understanding of a young person’s needs/challenges and signpost to appropriate mental health and community services.
* Assist a young person to build support networks to help maintain independence in the community.
* Liaise with colleagues and other professionals or agencies as required, including attending MDTs, planning meetings and transfer of care meetings.
* Confidently contribute to MDT discussions around plans for a young person.
* Work closely with mental health professionals and hold an individual caseload under the guidance of a qualified member of staff.
Qualifications and Requirements
* Excellent communication skills and an adaptable, non-judgmental approach enabling rapport and trust with young people who may not ordinarily access help.
* Comfortable having conversations and persevering with a young person about needs, worries and engagement barriers. Lived experience is advantageous.
* Excellent knowledge of mental health and community services available for young people across Surrey.
* Note: Lone working is involved in this role while working with complex young people.
* Ability to work with families in a way that is helpful to the young person.
* Must hold a current driving license and have access to a car for work purposes.
* IT skills including ability to use a computer.
* Ability to accurately and concisely document notes and risks onto the system after sessions.
* Ability to conduct risk assessments, identify changes to clinical risk and implement actions to ensure a person’s safety.
* Knowledge of responsibilities in relation to the Children’s Act.
* Knowledge and awareness of safeguarding procedures and experience of developing a plan of care to safeguard a child or adult.
* Active role in hub functions including co-facilitation of groups for CYP and families, participation in navigation and screening meetings, and being part of the multidisciplinary team.
