Vitalis are recruiting for an experienced Social Worker to work in a Hospital Team in Barnsley Council.
We have an exciting opportunity for a qualified Advanced Social Work Practitioner to join our Hospital Social Work Team, working in close partnership with health professionals across various discharge pathways.
What we’re looking for:
1. Essential:Hospital social work experience within adults and strong knowledge of discharge processes.
2. Essential:Experience managing and supervising social work staff.
3. Skilled in collaborative working with health partners to ensure safe, timely discharges.
4. Strong IT skills, including Liquid Logic and Excel.
5. In-depth understanding of:Mental Capacity Act 2005Human Rights Act 1998Care Act 2014
6. Ability to work with complex needs such as substance misuse, homelessness, mental capacity issues, self-neglect, and advocacy.
7. Confident in staff supervision, decision-making, and caseload management.
8. Ability to undertake Care Act, Capacity, and Best Interest Assessments when required.
