We are currently seeking an experienced Advanced Nurse Practitioner(ANP) to join our team as a Home Visiting ANP. This position offers a uniqueopportunity to provide high-quality medical care to patients who may facechallenges accessing traditional healthcare settings.
Main duties of the job
- Conduct comprehensive home visits to assess patients'medical needs, provide accurate diagnoses, and develop individualised treatmentplans tailored to their unique circumstances.
- Perform routine medical examinations, screenings, andnecessary diagnostic tests to ensure timely and accurate assessment ofpatients' health status.
- Prescribe medications, administer treatments, and ensureany appropriate onward referrals to optimise patients' health outcomes.
- Educate patients and their families about their healthconditions, available treatment options, and preventive measures to promoteoverall well-being.
- Collaborate closely with a multidisciplinary team ofprofessionals, including GPs, Nurses, Occupational Therapists, Mental HealthPractitioners, Social Workers, Housing, 3rd Sector/Voluntary andmore to ensure seamless coordination of care and comprehensive support forpatients.
- Maintain meticulous and up-to-date medical records incompliance with regulatory standards and best practices to ensure continuity ofcare and effective communication among healthcare providers.
- Demonstrate empathy, sensitivity, andprofessionalism in all interactions with patients and their families, fosteringtrust and confidence in the care provided.

Job responsibilities
An exciting opportunity has arisen for an experienced ANP to join ourHome Visiting Team, working in and around Boston. You will be an enthusiasticand motivated ANP who wishes to influence patient care in this bespoke area ofprimary care.
Your main duties will be to assist daily with home visits including visittriage, seeing on the day patients for acute and minor illness, working onbehalf of Boston Primary Care Network (PCN) practices.
You will be employed and supported by Boston PCN Ltd and will work with adiverse, multi-skilled, experienced professional team of clinical andnon-clinical, friendly and committed staff.
Boston PCN covers six practices across Boston, Kirton and Swinesheadareas.

Essential

14. Registered with NMC as RGN, with NMP qualification and evidence of masters level education for advanced clinician practice or working towards, including advanced examination skills
15. Experience of working within a demanding primary care clinical setting, preferably GP practice.
16. Independent prescriber
17. Full and clean UK Driving Licence and access to transport.
Desirable
18. Experience of working in a community health / home visiting care setting linked to GP Practice.
19. Experience of working in a multi-discipline health care team.
