As part of our ServiceRedesign, new posts have been developed to support our high risk patients whorequire a fast response in order to prevent hospital admissions where possible.This is an exciting opportunity for an innovative and experienced therapist tojoin the countywide Worcestershire Adult Speech and Language Therapy (SLT) Service,which covers Community Hospitals, Out-patient Clinics and Specialist Teams,with the aim of providing the optimum evidence-based service for the people ofWorcestershire.
Full time Countywide post (part time considered; minimum WTE) will focus onFast Response under the guidance of a Highly Specialist Clinical Lead, as wellas working alongside experienced community colleagues. Clinically, you willprovide specialist assessment, diagnosis and case management for patients witha range of communication disorders and dysphagia related to a variety ofdiagnoses and may maintain a small caseload.
You will be integral tothe continued development of the Service into a high performing, well respectedprovider of timely, safe and effective Therapy for ourpatients, therefore we require an ambitious and creative clinician who isself-driven, and who also works well within a team.
You will be supportedwith excellent management and clinical supervision in this post, which is basedin the north of the county.
We are happy tonegotiate flexible working hours to fit with individual needs as well asfulfilling the requirements of the Service.
Main duties of the job
Atypical day will involve managing a caseload of fast response referrals inliaison with the Clinical Lead, prioritising visits across the county, raising anypotential concerns regarding capacity and demand. You may also maintain a smallgeneral caseload based in the north of the county, working at a Specialistlevel.
Youwill work independently in the assessment and management of patients withcommunication and swallowing disorders, completing clinical documentation onour electronic records system.
Youwill formulate individual therapy programmes targeting realistic patient-ledgoals, as well as offering support and education to carers/families.
Youwill liaise with other members of the team and MDT colleagues; support studentplacements; provide in service training for SLT colleagues and otherprofessions in areas related to SLT, and participate in audits as required bythe service and the Trust.
Youwill attend clinical and service meetings.
Youwill receive training and supervision, and will engage in self-directedlearning.

10. Attainment of formal dysphagia competencies allowing independent assessment, treatment and full case management with this client group
11. Attainment of RCSLT NQP Competencies Framework allowing full membership of RCSLT
