We are looking for an experienced Short Breaks Coordinator to lead the coordination of an efficient, accessible and cost‑effective short break service that enhances the lives of children and young people with Special Educational Needs and Disabilities (SEND).
The role is based within the Children’s Specialist Support Team and works collaboratively with all children’s social care teams. The Coordinator will plan and deliver short break services in partnership with agencies, families and professionals. The position may include travelling within the local authority to meet service users and partners.
Qualifications
* Degree level education or equivalent.
* Evidence of ongoing professional development to meet the requirements of this role.
* Experience working with children and young people with SEND is desirable.
Responsibilities
* Work closely with specialist support team and other agencies to ensure short breaks are integrated with other services, allocated fairly and within policy and best practice guidelines.
* Collaborate with children, young people, families, schools, health professionals and other partners.
* Maintain, develop and deliver a Short Breaks Sufficiency Strategy that focuses on services that need to be commissioned to meet the diverse and often complex needs of children and young people with disabilities, in consultation with children and their families.
* Develop policies and procedures for the Short Breaks Service in line with best practice and corporate guidelines.
