Community Mental Health Nurse - Intensive Support Team
We have a great community nursing opportunity within our Intensive Support Team (IST), at High Weald, Lewes and Havens Specialist Older Adults Mental Health service. This post will suit a Registered Mental Health Nurse (we will consider Learning Difficulties nurses who have sufficient experience working with Dementia or Severe Mental Illness).
This role involves working with older people with complex mental health and physical health needs. You will support both clients and their carers, collaborating with internal services and partner agencies.

The position is based at Millwood, Uckfield, offering 37.5 hours per week, including weekend work.

Main duties of the job
The post holder will be an autonomous practitioner responsible for a defined caseload, undertaking:
1. Assessments of individuals and families with complex health presentations, including high-risk cases, developing care plans with service users.
2. Providing specialist nursing assessments and advice as needed.
3. Assessing and managing ongoing risks collaboratively with service users and carers, reviewing and adjusting risk management strategies regularly.
4. Supervising junior staff and trainees where appropriate.

Person Specification
Qualifications
* Registered Nurse (mental health/learning disabilities/adult), current registration
* Completion of Mentorship Course/ENB equivalent
* Evidence of continuous professional development
Knowledge/Experience
* Significant post-qualifying experience in the relevant care group
* Experience working with people with complex health issues
* Experience providing clinical interventions for diverse health problems
* Experience working consultatively with professionals
* Experience conducting clinical and risk assessments
* Experience in partnership working across statutory and non-statutory agencies
* Experience within the CPA process
* Understanding of clinical governance
* Previous community-based experience
