This role is for a Team Leader - Nights at Avery Healthcare's Wandsworth Common Care Home in London. The successful candidate will be responsible for supporting the home management team in delivering high‑quality, person‑centred care to the residents.
Main duties of the job
As a Team Leader - Nights, your focus will be on providing a safe, caring, innovative, open, supportive and inclusive environment for residents, visitors and staff.
Job responsibilities
* Lead the care planning process, including implementing, monitoring and undertaking monthly evaluations, completing care plan audits and developing action plans.
* Assist with resident discharges, ensuring proper documentation, medication management and keeping next of kin informed where appropriate.
* Monitor the well‑being of each resident, identify changes impacting care, update care planning and amend written records accordingly.
* Practice safe systems of work across tasks and moving and handling of residents, assessing risks and ensuring the safety of residents, visitors and staff.
Qualifications and experience
* NVQ 3 or equivalent in care for care homes.
* Two years’ experience in a senior position with the resident group.
* Strong communication skills, both verbal and written.
* Ability to lead staff and act responsibly in the absence of the manager.
