Key Responsibilities
* Deliver evidence‑based, high‑quality clinical care that focuses on the direct needs of the patient.
* Assist registered nurses and practitioners with delegated care of patients and contribute to the holistic needs of individuals.
* Work independently and as part of a multidisciplinary team, supervising patients and assisting with future care plans.
* Provide day‑to‑day care for service users under the supervision of a clinician, adhering to best practice, standards and guidelines.
* Support the induction of junior healthcare workers and apprenticeships.
* Collaborate with service users, other MDT staff and external agencies to achieve optimal health outcomes.
* Maintain the dignity, equality, privacy, respect and confidentiality of all clients.
Requirements
* Positive attitude to mental‑health care of older‑persons client group.
* Ability to ensure privacy, respect, dignity and confidentiality are maintained at all times.
* Willingness to work as a Band 3 Health Care Support Worker in the Denbighshire Community Mental Health Team.
* Welsh language skills desirable; Welsh and/or English speakers are equally welcome to apply.
