About the role:

We are seeking a dedicated, enthusiastic, and flexible individual to join our team as a Work and Health Coach within the new Trailblazer Project.

The Trailblazer Programme is an ambitious, multi-agency initiative led by Neath Port Talbot Council to reduce economic inactivity and improve wellbeing. We deliver innovative projects in Opportunity Hubs and outreach locations, working with health professionals, training providers, and employers to help people overcome complex barriers to work, including mental health, health conditions, digital exclusion, housing instability, and low confidence.

You will join a passionate, supportive, and forward-thinking team that values collaboration and kindness. We celebrate success, share knowledge, and are driven by the positive impact on people's lives.

As a Work and Health Coach, you will provide tailored, one-to-one support to individuals facing significant barriers to employment, training, and wellbeing. Using a holistic, person-centred approach, you'll help participants set goals, build confidence, develop skills, and access services to move towards sustainable employment or improved quality of life. You will manage a varied caseload, work closely with partner organizations, and support individuals from initial engagement to in-work support, ensuring they have the tools and confidence to succeed.

Your daily routine will include:

* Meeting participants for coaching sessions to review progress and update action plans
* Conducting initial assessments and developing Individual Action Plans
* Liaising with partner services such as health teams, training providers, and employability programs
* Recording case notes and updating the digital case management system
* Providing in-work support to help participants settle and sustain employment
* Coordinating access to wellbeing, training, or community resources to remove barriers and build confidence

About you:

Do you have:

* Experience delivering one-to-one coaching, mentoring, or wellbeing support
* Proven ability to engage individuals with barriers such as poor mental health, disability, or social isolation
* Skills in building trust and motivating using strengths-based approaches
* Ability to adapt communication to various needs and audiences
* Organizational skills to manage a caseload and maintain accurate records
* 4 GCSEs grades C or above, or equivalent (e.g., Intermediate GNVQ Level 2, BTEC First Certificate, NVQ Level 2)
* Level 4 in Advice and Guidance or employability-related qualification, or working towards it

Preferred experience in roles such as:

* Employability Adviser or Job Coach
* Health and Wellbeing Coach
* Support Worker (mental health, disability, or community support)
* Social Prescriber or Link Worker
* Advice and Guidance Worker
* Community Development Officer
* Rehabilitation or Occupational Support Worker
* Case Manager within health, social care, or employability services
* Training or Skills Development Officer
