Overview
The post holder will assist in the provision of an efficient, effective and comprehensive administrative service supporting the Integrated Discharge Team and Enhanced Care Team. The post holder will be expected to work alongside ward staff/Therapies/Patient flow team as well as external partners to facilitate discharge flow and the organisation of Enhanced Care services and support achievement of key performance indicators. Using appropriate technologies and equipment the post holder will produce all work outputs to a high standard meeting the organisations requirements for Patient Safety, Information Governance and CQC recommendations in adherence with Trust policies, procedure and guidelines.
We are excited to offer an opportunity within Patient Flow and the Integrated Discharge Team based at UNHT. You will work closely with the ward staff as well as local partners including social care and voluntary agencies to support care transitions.
Responsibilities

* Excellent communication skills and confidence working across professional boundaries.
