Job Overview

This is a Fixed Term/ Secondment post until 31/03/2027 due to funding.

We are looking for an enthusiastic and dynamic dietitian to join our well respected community dietetic team in Pembrokeshire as an experienced dietitian for community and integrated care.

This post is a 14 month fixed term/secondment post and offers an ideal opportunity for a dietitian looking to develop their clinical skills, or an existing experienced dietitian that would like to specialise in urgent and integrated community dietetics. The post-holder will fully embed themselves as part of a wider MDT, supporting embedding nutrition/hydration as part of models of care and driving forward innovation. The post-holder base is Pembrokeshire and we welcome applicants who would like to discuss flexible working opportunities, in terms of the way the hours are worked.

Main duties of the job

Acting as experienced dietitian for the community urgent integrated care team, working collaboratively with proximal community multidisciplinary services, to strengthen communication and clinical delivery across community settings.

You will be managing a varied caseload of dietetic referrals for adults with nutritional support needs; working as part of the community dietetics team, with strong connections with the acute dietetics team. You will provide specialist dietetic advice, in relation to nutritional management and produce a coordinated care plan.

Supporting, embedding and strengthening the nutritional screening at the 'front door', you will be part of rapid response pathway and models of care, including providing training and education; accredited training to therapy assistant practitioners and others; working as part of the front door model to de-escalate risk.

You will support the wider community nutritional support team, by providing supervision and training alongside other experienced dietitians, ensuring, effective prioritisation of referrals and that dietetic pathways and processes are fully embedded.

The role will ensure you deliver dietetic care in a variety of settings to meet the needs of the patient, including undertaking home visits, attending care home/residential settings, community hospitals and providing group education and working across virtual platforms.

Two integrated care centres: Aberaeron and Cardigan in Ceredigion, and several other community settings
47 general practices (six of which are health board managed practices); dental practices (including four orthodontic); 97 community pharmacies; 43 general ophthalmic practices; and 8 ophthalmic domiciliary providers
Numerous mental health and learning disabilities services
