 
        
        We have an exciting opportunity for a Long Term Conditions Nurse to join our Primary Care team at HMP Full Sutton, a category A men’s prison located in Full Sutton, York.
This rural site offers its own parking, an on‑site gym, café, team building days and well‑being events each year, regular 1‑1’s and a strong scope for development including vaccination training and opportunities to gain advanced qualifications.
Working Hours & Salary
 * 30 hours a week – 4 days a week, hours negotiable.
 * No weekends, ideal start 08:30‑09:00, finish 16:30‑17:00.
 * Full‑time role with an annual salary up to £49,175 FTE, dependent on experience.

Key Responsibilities
 * Ensure comprehensive handover of services when patients transfer across wings or custodial care settings.
 * Identify immediate health needs and any external services or prescribing requirements prisoners may have.
 * Facilitate arrangements between agencies to deliver complex care packages, working jointly with in‑house health services and prison departments such as Pharmacy, Education, Offender Management, Substance Misuse and Physical Education.
 * Provide proactive intervention to urgently address improvement recommendations from CQC and HMPP inspections.
 * Deliver continuity of care across health and social care agencies from courts to custody and community.
 * Inspire continuous improvement of MDT working across HMP and healthcare teams.
Qualifications & Experience
 * Registered Nurse – General with current NMC registration.
 * Experience in long-term conditions.
 * Advanced Nurse Prescriber/Practitioner – desirable.
 * Degree level education.
 * Knowledge of health and social care models across the criminal justice system.
 * Knowledge of Evidence Based Practice and Clinical Governance.
 * Experience assessing and working with individuals with complex health and social care needs.
Although not essential, experience in a hospital, community or prison environment is advantageous.
