We are currently looking for 2 female health care assistace who have the below skill set:
Package of Care
* POC - QDS x2 carers
* Call pattern: 120 mins / 60 mins / 60 mins / 60 mins + 60 mins
* Total weekly hours: 84
Package Summary
AR is tetraplegic following a spinal cord injury and requires assistance with all daily living needs. Her needs are intense and complex, with key risks including breathing concerns, skin breakdown/pressure damage, UTI/constipation, falls, autonomic dysreflexia, and deterioration if care is not delivered consistently.
Agency Must Provide Carers With The Following Skills/Experience
* 2:1 care support
* Medication administration
* Suprapubic catheter care, including connecting/disconnecting night drainage and monitoring for blockage
* Bowel care including digital rectal check, administration of glycerine suppositories, digital stimulation / manual evacuation
* Autonomic Dysreflexia awareness and symptom management
* Blood pressure monitoring and ability to act on readings / elevate concerns appropriately
* Hoist transfers and safe repositioning
* Pressure area care / skin checks
* Splint application/removal
* Physiotherapy exercises support
* Inspiratory Muscle Trainer / cough assist awareness
Call Details
AM call - 120 mins
* Light breakfast, hot drink, medication
* Disconnect night drainage bag from suprapubic catheter
* Remove upper and lower limb splints and check for marking
* Full skin check, including pressure areas
* Digital rectal check for faeces
* Insert 2 glycerine suppositories if required
* Digital/manual evacuation
* Sit upright for breakfast and assist as required
* Exercises
Lunch call - 60 mins
* Administer prescribed medication
* Prepare lunch and drink, assist as required
* Hoist transfer as required
* Exercises
Tea call - 60 mins
* Prepare evening meal and drink, assist as required
* Medication support
* Wheelchair repositioning / hoist transfers as needed
* Exercises
Evening call - 60 mins
* Full assistance with undressing
* Remove abdominal binder
* Thorough skin check
* Personal hygiene / teeth brushing
* Assist with drink if needed
* Position for bed
* Connect catheter night drainage bag
* Administer medication
* Check BP as required
* Turn to preferred side and check pressure areas
* Apply night lower and upper limb splints
* Settle for the night
Additional Notes
AR has a suprapubic catheter and neurogenic bowel/bladder dysfunction
She is at risk of Autonomic Dysreflexia, and BP monitoring plus prompt intervention are important parts of care
She requires repositioning, pressure relief, and close observation of skin integrity
