We are lookingfor an Advanced Clinical Practitioner who has extensive clinical experience in minorinjuries and illness, in an urgent and/or Primary Care setting. The candidatewill possess a completed relevant MSc/ or credentialled in advanced practicethat will enable them to work as part of the medical team, seeing a full rangeof adult and paediatric, acute and urgent conditions.

Pay: £27.49 - £30.63per hour depending on experience

**Golden hello £2500 offered paid in two instalmentsover first 6mths pro rata**(pro rota thisapplies to EXTERNAL APPLICANTS ONLY)

Hours:Negotiable - Full and Part time hours are available. Full time is 37hours a week


Main duties of the job

You will workwith our friendly, supportive, ever-expanding team of clinicians here at FCMSUrgent and Emergency Care, Yorkshire. To deliver high quality, NHS care to thepeople of Doncaster, 24 hours a day, 7 days a week. The post holder will beexpected to work across all areas including but not limited to the UrgentTreatment Centre (UTC), including the front desk streaming in Doncaster RoyalInfirmary, the Same Day Health Centre (SDHC), the ECP home visiting service andthe OOH triage services. The services we run allow patients to access same dayhealth services and support NHS services across Doncaster, serving a populationexceeding 300,000. Job responsibilities

Keyareas of responsibility will include but are not limited to:

Clinical

The postholder will:

* Assess diagnosis, plan and implementprogrammes of evidenced based care for the client group, undertakingre-evaluation and implementation of treatment plans as appropriate.
* Ensure interventions are undertaken in linewith best practice guidance.
* Undertake advanced skills appropriate to therole, ensuring clinical competencies are maintained in line with individualgoverning bodies and the Advanced Practice Framework.
* Provide advice, information, knowledge andsupport to staff.
* Identify local, demographic, cultural andenvironmental factors that influence the health and quality of life.
* Utilise non-medical prescribing qualificationadhering with local formularies, policies and Clinical Governance To provideappropriate non-medical prescribing support for the organisation in adherenceto Clinical Governance frameworks and in adherence to the agreed policy.
* Continue to evidence the four pillars ofadvanced clinical practice.
* Undertake annual audit of prescribingpractices.
* Ensure that child protection and vulnerableadult legislation is adhered to within their own practice and the service.
* Assist with the development and evaluation ofpolicies/guidelines/protocols for advanced and specialist clinical practice
* Continually review and evaluate own role andclinical practice informing the annual review process with the Clinical Lead.
* Plan and manage their own workload and theworkload of the service, in a flexible manner, ensuring that service provisionmeets the needs of patients.
* Promote a positive relationship with allstakeholders within the service.
* Contribute to the clinical governance process,including adverse incident reporting/investigation and the management ofcomplaints, as appropriate.
* Be involved in peer review of own practice andthat of others.
* Be flexible to support the need of theservice, across both sites and the home visiting service.

Safeguarding

Safeguarding is everyones responsibility, and all employees arerequired to act in such a way that at all times safeguards the health andwell-being of children and vulnerable adults. Staff must be aware of theorganisational procedure and raising concerns about the welfare of anyone andwhom they have to contact. To fulfil these duties, you will be required to attendtraining and development to recognise the signs and symptoms of abuse orindividuals at risk and to report and act on any concerns you may haveappropriately.

Professional / Leadership

The post holder will:

Role model standards of care and behaviourthrough clinical practice.

Analyse relevant national policy, within theirarea of expertise and advise on the impact for service, leading theimplementation of changes and evaluation of processes.

Maintain professional registration andpractice through CPD.

Maintain a professional portfolio todemonstrate competency in line with the advanced practice framework.

Maintain personal standards of conduct andbehaviour consistent within FCMS and their independent governing bodys guidelines.

Ensure appropriate use of resources to meetservice needs.

Recognise and act as an advocate for patients,carers, the service and the organisation.

Lead change as required and ensure resolutionof conflict that may occur.

Promote workforce integrity (empowerprofessional accountability within the clinical team, ensure accreditationprocess followed, complaints management, competency assessment, ensuringmandatory training requirements fulfilled, robust clinical induction)

Participate in regular clinical supervision.

Ensure CAS & NICE alerts are cascadedappropriately.

Maintain a sound knowledge of NHS policy andNICE guidance in relation to Urgent Care.

Non-Medical Prescribing

You will need topossess prescribing capacity and must have a current registration with yourprofessional body that includes V300 qualification. You must only prescribefrom an agreed Personal Formulary which reflects your competency and experienceand is aligned to up to date evidence / best practice guidance. You will beexpected to review and develop your Personal Formulary on an annual basis.

Education and training

The post holder will:

Holdan MSc in Advanced Clinical Practice or Equivalent with Level 7 Qualificationsand be Credentialed.

Beresponsible for identifying their own development needs in line with servicerequirements and a personal development plan.

Develop new ways to improve service and leadwith changes. Provide clinical supervision both within the organisation tosupport the development of individuals and the service.

Ensure ACP clinical supervision training iscompleted

Ensure the clinical environment is conduciveto supporting the education and learning of all staff and students

Provide educational and training opportunitiesto students to ensure placements satisfy the relevant elements of theirlearning agreement

Contribute to clinical governance outcomes.

Understand and apply professional and nationalguidelines on confidentiality.

Be fully involved in the progress anddevelopment of clinicians and provide constructive and relevant feedback.

Be a Designated Prescribing Practitioner fornon-medical prescribing students

Offer face to face clinical supervision

Research and development

The post holder will:

Evaluate clinical practice through audit ofservice and develop and manage strategies to address any shortfalls.

Take part in organisational teaching sessions

Demonstrate knowledge of current research inall aspects of their work and advise others on the implementation of relevantresearch findings.

Initiate and/or become involved in researchprojects in response to identified needs

Disseminate results/research findings vialocal teams and/or national/international publications.

Implement research findings appropriate topractice.

Promote the shared objectives of themultidisciplinary team by working closely to ensure that best practice isachieved

Communication and working relationships

The postholder will:

Communicates effectively with other teammembers

Take all appropriate steps to minimise conflictand deescalate situations

Clearly document all contacts with a patientand refer on as appropriate

The post holder will:

Refrain from thewilful misuse or interference with equipment provided in the interest of Healthand Safety

Report as soon asis practically possible any hazards or defects

Report as soon aspractically possible accidents or untoward incidents and ensure appropriateincident reporting documentation is completed

Be activelyinvolved as an appropriate departmental representative

Making effectiveuse of training to maintain knowledge and skills

Physical effort and working conditions

The post holder will:

Be responsible toensure all aspects of the Manual Handling guidelines are followed, providingappropriate risk assessments

Be responsible tomanage clinical care following required Policies and Procedures pertaining tobodily fluids, infection control and COSHH regulations resulting in frequentexposure to highly unpleasant working conditions

Be responsible tomanage challenging situations and people with some exposure to hazards, e.g.violent and abusive patients. Child abuse, critically ill patients

Manage own timeeffectively within variable shift patterns

Any other duties considered relevant to the post and commensuratewith the post holders grade and as agreed by their manager.

Qualifications

* Relevant and live Health Care Professional Qualification and registration
* MSc Degree in Advanced Practice or Equivalent Level 7 Qualifications and Credentialling.
* Experienced Non-Medical Prescriber
* Qualification and experience in assessing and treating minor to moderate injuries/illness
* Teaching and mentorship qualification / certificate
* Relevant Professional Qualification / registration
* Speciality qualification


Other

Experience

* Experience of working independently as an unsupervised practitioner in the primary or urgent care setting.
* Portfolio of evidence showing achievements in practice/speciality
* Evidence of excellent clinical record
* Maintained development of clinical leadership skills
* Evidence of significant involvement in change
* Demonstrates ability and knowledge on how to undertake an audit and received feedback as a clinician
* Awareness of impact of evidence-based practice within Urgent care.
* Experience of Emergency department triage / streaming
* Demonstrate the ability to work in highly demanding environment
* Evidence of diplomacy and negotiation skills
* Able to demonstrate imaginative and effective use of resources

* Competent in use of IT packages such as Microsoft Word and PowerPoint

* Experience of working with clinical IT systems including Adastra, System 1 and EMIS.
