With experience in palliative care, you're ready and able to make a real difference. You will work in close collaboration and under the line management of the senior nurses in the service to provide direct support to patients and their families who are newly identified as having a palliative diagnosis. The role will include making first contact, arranging follow-up, and offering signposting to support services and advance care planning discussions, so excellent communication skills are a must.
