With an increasing emphasis on community-based care there has never been a better time to join our team. We are Kent’s leading provider of NHS services in the community and we are looking for a highly motivated respiratory nurse to join our specialist respiratory team in East Kent.


Responsibilities

* Work alongside an established multi-disciplinary team and hold a caseload of complex patients with a range of respiratory conditions that require specialist input.
* Autonomously manage the assessment, planning, delivery and evaluation of patient care and utilise a whole systems approach when supporting admission avoidance for this client group.
* Deliver excellent patient care along a care pathway across organisational boundaries.
* Act as a specialist resource and provide education to the wider multi-disciplinary team across the health economy.
* Develop and implement management plans for patients who have complex respiratory conditions.


Working Pattern and Location

In return for your hard work we can offer you a flexible working environment, personal development and a comprehensive corporate and local induction programme. The post is for 22.5 hours per week and we will consider a band 6 or a band 5 development post if a passion for respiratory and a drive to make a difference are demonstrated in your application and interview.

We currently have bases in four localities across East Kent and whilst the successful applicant will be based in the Thanet locality, they will be expected to be flexible in their approach and travel to other areas within East Kent when required. Applicants must be able to drive and have access to a car for use as part of their role.

Additional Working Pattern: 22.5 hours between Monday - Friday; pattern to be agreed.

As a community Trust many of our roles require individuals to be able to drive to perform their duties.
