This is a venture supporting clinicians & allied healthcareprofessionals across the four practices in Selby Town PCN.
To addressthe ever-increasing demand for primary care services there is a recognition ofthe need for better support for clinicians within practices whilst at the sametime undertaking work in connection with Population Health Management. Selby TownPCN have a proven track record of developing new roles introduced as part ofPrimary Care Networks (PCNs) & this role is new to the PCN. Our locality network is committed to developinga multidisciplinary workforce which it sees as essential in providing a highlevel of care to its population.
Main duties of the job
The GPA will support PCN staff, including the Dietician & Health& Wellbeing Coach in the smooth running of the services they provide. Thismay include performing the more routine administration and clinical tasks.

The award-winningSelby Town PCN consists of four practices with a combined population ofapproximately 50,000 patients, covering the large geographical area surroundingSelby and Escrick.
The PCN worksover four practice sites (Beech Tree Surgery, Escrick Surgery, PosterngateSurgery and Scott Road Medical Centre) our local community hospital & The Summit, an ex indoor adventure park. The PCN has a Clinical Director, two ClinicalLead GPs and is managed by the PCN Operations Manager. We work in increasinglycollaborative partnerships across a number of workstreams, which includeimportant priority areas of mental health, frailty, urgent care and healthinequity. Our team consists of a diverse multi-professional workforce whichincludes pharmacists, MSK practitioners, mental health workers, carecoordinators, social prescribing link workers, a frailty therapy team, anurgent care coordinator, ANPs, paramedics, ACPs, a transformational lead, adigital lead, a dietitian, a health & wellbeing coach & an HR Lead.

KEY DUTIES
Running searches to identify patients forspecific groups.
Sending bulk text messages to invite patientsto groups.
Updating patient records with codes afterattending groups.
Follow up information sent to patients viaemail.
Room set-up
Welcoming patients.
Taking routine measurements such as height,weight, BP as/when
required.
Monitoring shared email inbox.
General monitoring of activities taking placeat the Summit.
