We are redesigning our community services model and implementing a new Intermediate Care Team. An exciting opportunity has arisen to appoint General Therapy Assistant Practitioners to help deliver urgent community response services. These roles will be crucial in developing the service, providing essential support and care to individuals, with a focus on promoting independence, dignity, and well-being.
The objective is to ensure safe and timely discharge from inpatient stays for patients needing continuing care or to prevent admission with interventions manageable at home.
The post holder will work within the Cwm Taf Morgannwg University Health Board Intermediate Care Service, supporting registered Occupational Therapists and Physiotherapists in delivering interventions to patients in their homes and community settings. Responsibilities include managing a caseload of non-complex patients, delivering prescribed interventions under supervision, and working independently within standard operating procedures, referring to qualified professionals when necessary.
The role involves working as part of a multidisciplinary team, liaising with stakeholders such as Local Authorities and the third sector, assisting with therapeutic care modalities aimed at expediting hospital discharge and maintaining patients at home, and preventing avoidable hospital admissions. Tasks include environmental assessments, fitting assistive equipment, and participating in service evaluation and data collection to demonstrate outcomes. Promoting independence, health, and wellbeing is central to this role.
Desirable Welsh language skills include speaking and listening at Level 3 (CEFR B2) or above, which involves basic conversations about health. This is not essential but considered an advantage during shortlisting and selection.
