We have an exciting opportunity for motivated, committed and experienced dietitians to join our bank staff team. You will ideally have at least 2 years prior experience working in an acute hospital setting with relevant registrations and qualifications including HCPC. The ideal candidates will have experience gained through training and post registration experience, including but not limited to one or more of these clinical areas:
1. Oncology inpatient and/or outpatient settings
2. General surgery
3. Critical care
4. Outpatient clinics (gastroenterology / nutrition for health)
5. Group education (i.e. cardiac rehabilitation)
As a Dietitian you will:
6. Ensure accurate clinical records are maintained in accordance with professional, statutory and Nuffield guidelines.
7. Produce/adapt equipment to meet patient needs
8. Undertake discharge planning
9. Evaluate relevant information to plan the range and sequence of assessment required and determine the specific activities to be undertaken, the risks to be managed and the urgency with which assessments are needed
10. Select appropriate assessment approaches, methods, techniques and equipment, in line with individual needs and characteristics, evidence of effectiveness, and resources available
11. Explain clearly to patients their own role, responsibilities and accountability, and the information that is needed from the assessment and who might have access, respecting peoples dignity, wishes and beliefs; involving them in shared decision making; and obtaining their consent
12. Prepare for, carry out and monitor assessments in line with evidence based practice, and legislation, policies and procedures and/or established protocol/established theories and models
13. Monitor individuals during assessments and takes the appropriate action in relation to any significant changes or possible risks
14. Evaluate assessment findings/results and takes appropriate action when there are issues
15. Consider and interpret all of information available using systematic processes of reasoning to reach a justifiable assessment, explaining the outcomes to those concerned
16. Determine and record diagnosis and treatment plans in accordance with professional guidelines
17. Identify individuals whose needs fall outside protocols/pathways/models and makes referrals to the appropriate practitioners with the necessary degree of urgency
18. Identify new developments made by others that might be relevant to own area of work to determine if and how they could be applied within own area of work
19. Propose the adoption of relevant developments within own work area to relevant decision makers
