Gtd healthcare is one of the largest NHS-commissioned Primary and Urgent care providers in North West England. We have an excellent opportunity for a Care Co-ordinator to work across both sites of our Oldham Urgent Care Service on a part-time basis. The service includes the Urgent Care Hub based in the Integrated Care Centre and the Primary Care Streaming Service in the Royal Oldham Hospital.
As a Care Co-ordinator, you will be the first point of contact for healthcare professionals, patients, and relatives accessing urgent and non-urgent care services in Oldham. Under the supervision of the Senior Care Co-ordinator and Clinical Shift Lead, you will register patients onto the relevant clinical system and direct them to the most appropriate service or pathway, supporting a 24/7 care co-ordination service.

Working hours are 23 hours per week, Monday to Friday during core hours.
Responsibilities include:
* Delivering high-quality healthcare coordination to patients and professionals.
* Accurately recording data on clinical systems and directing patients appropriately.
* Booking appointments across healthcare services.
* Collaborating with the Senior Care Co-ordinator and Clinical Shift Lead to manage patient flow and prevent excessive waiting times.
* Coordinating follow-up care as directed by the clinical team.
* Working with the multidisciplinary team to ensure smooth service operations.
* Communicating with NHS 111 regarding patients out of area, duplicate calls, or incomplete information.
