An opportunity has arisen to join the Patient Flow andDischarge Team at Barnsley Hospital.
We are looking to recruit a Patient Flow and DischargeAssistant to help support the smooth running of all aspects of patient flow forthe site as well as assisting with the daily co-ordination of the discharge hubsupporting patient discharges through the identified discharge pathways.
This is a fast paced administrative role working within abusy environment alongside a supportive team.The ideal candidate will be a team player with a motived positiveattitude. You must have excellentcommunication skills with a willingness to learn new skills and have someunderstanding of safe patient flow and discharge processes.

We are looking to recruit an enthusiastic and motivatedindividual to join the Patient Flow and Discharge team here at BarnsleyHospital.
The Role
This interesting and busy role hastwo elements to it:
Patient Flowsupports the Site Matrons withfacilitating safe and effective patient flow, bed movements and dischargeswithin the Trust, supporting the daily bed meetings, data gathering and inputting,submitting external SITREPs to NHS England, completion/updating of e-forms etcand producing system access for NHSP staff.
Discharge supports the HomeFirst Review team and involves working alongside team members to ensure safedischarges are followed by the ward, co-ordinating incoming/outgoing telephonecalls and email communications between wards/departments, recording dataaccurately and liaising with external partners.
It is importantthat candidates have excellent communication skills, a willingness to learn newskills and have some understanding of safe patient flow and discharge processes. Candidates must have the ability to follow instructions and work on their owninitiative when required.

Qualifications
* At least 4 GCSEs
* (Grade A C)
* NVQ Level 3 or willingness to work towards
* In around 100 words please describe what Equality & Diversity means to you and why they are important.

Experience
* Previous experience of working in a hospital setting
* Previous experience of using CareFlow
Knowledge and Awareness
* Able to organise own work and use own initiative
* Sound understanding of confidentiality
* Basic understanding of clinical governance
* Understanding of patient flow
