We have an exciting opportunity in Geriatric Divison care of the elderly patient in the Frail Older Person Liason Service KGH site at BHRUT Hospitals. We are looking to recruit a Clinical Band 6 to join our existing highly motivated team on the FOPALS at KGH for a year fixed term to cover Career break. This post is at FOPALS is based at KGH Hospital Site. Care of the Elderly is currently going through a process of change to ensure we meet the on-going needs of our patients and to provide an excellent standard of care every time every day. This is an ideal opportunity for you to bring your experiences and visions to help us to deliver these changes. We are looking for an enthusiastic nurse with a passion for delivering high quality care, training to our staff in partnership with patients and their relatives putting the patients at the centre of our PRIDE values and responsibility for this area of nursing and maintaining high standard of care and training. To provide professional leadership, support and advice to the nursing workforce within the FOPALS team ; and drive developments in nursing practice, to improve patient care and experience. The role will facilitate education, and training for registered and non-registered nursing staff; as well as act as a role model, and to provide educational and training support in the clinical area for all aspects of nursing.
The nurse will be working in a busy Emergency Department which operates 24/7, the nurse will be expected to assess patients from London ambulance service, Gp’s, Community Treatment Team (CTT), and also to identify Frail and elderly patient in the Accident and Emergency department to promote flow and good patient experience, avoiding unnecessary overcrowding in ED.
The nurse will be expected to deputise in the absence of the Team Leader in staff management and governance manner, to work alongside other band 6 nurses and unit lead to contribute to the overall management of a defined case load of patients with varied nursing needs and in addition support those with palliative needs and understand and comply with the control drug Act regulations administering IV medications when certified under 'Extended role of the nurse' within the NMC regulations.

Be a principle driver in the teaching and progress of projects identified from national initiatives and the Trust’s Nursing & Midwifery & Quality Strategies To work in conjunction with Acute care of the elderly nursing teams in the assessment and audit of staff clinical skills. To collaborate with Team LEAD in working with staff who are under performing, providing objective assessment and reviewing performance.
To provide written reports in relation to individuals ability to perform at the required level and in accordance with the Nursing and Midwifery Councils Code of Conduct To provide professional nursing advice and guidance and to participate in activities to fulfil the Trust’s Quality and Safety Agenda.
Main duties of the job
As a Band 6 post holder, you will work collaboratively alongside Emergency Care doctors, specialist nurses, the Community Treatment Team (CTT), and the therapy team. Your role will focus on promoting holistic, family-centered care, acting as a strong advocate for frail older patients and their families. A key responsibility will be to assist with admission avoidance strategies at the Emergency Department front door.
Using evidence-based practice and a holistic approach, you will assess, plan, implement, and evaluate individualized nursing care for older patients within your scope of practice, ensuring that all possible avenues of care have been explored. You will be expected to set and maintain high standards of nursing care, demonstrating leadership and clinical excellence.
The post holder will utilise skills of clinical expertise, leadership, education, and management to ensure a seamless service for patients, carers, and staff. The post holder will provide advice, support, and information and will work closely with the multidisciplinary team to ensure a co-ordinated quality service.The post holder will be actively involved in ensuring a high-quality safe service for patient and staff leading on audit and research as appropriate.The skills and quality for the this role also includes: •Clinical leadership and decision-making skills.
