What you will do
* Collaborate within a multi-disciplinary team to ensure seamless hospital discharges and support plans.
* Conduct comprehensive care and support assessments tailored to individual needs.
* Undertake mental capacity and Best Interest’s assessments to safeguard client interests.
* Engage in vital safeguarding work to protect vulnerable individuals.
* Work with the transfer of care hub, prioritising and triaging for efficient patient support.
* Develop and review reablement plans to aid in the recovery and rehabilitation process.
* Provide essential support to carers, recognising their crucial role in the care process.
* Participate actively in decision support tool meetings to ensure comprehensive care planning.
Requirements of the Social Worker
* A degree or equivalent qualification in social work is essential.
* Current registration with Social Work England is required.
* Significant frontline experience within UK Social Services is necessary, demonstrating your competence as a qualified Social Worker.
