In your role, you will work closely with Occupational Therapists, Assistant Practitioners, support staff, clinical assessors (UCR team), district nurses, specialist nurses, and social workers to provide the best possible care to service users in their own homes.
This post also includes paid enhancements for weekends and bank holidays.
Please note applicants will be required to pay for their DBS check. Costs are deducted from salary following appointment. The cost of the DBS application is £26.40 (standard) or £54.40 (enhanced). This cost will be deducted from your salary over the first 2 months of employment.
You are encouraged to enrol for the DBS Update Service. An annual fee of £16 per year applies.
An exciting opportunity has arisen to join our developing integrated community services providing clinical triage, rehabilitation, falls management, and support for timely discharges from our acute Trust partners.
You will work autonomously as well as part of the multidisciplinary team, liaising closely with health and social care colleagues to coordinate services, promote recovery from illness, and enable health, wellbeing, and self-care. You will visit patients within their own homes using a goal-oriented focus to maximise their independence, safety, and quality of life.
The post will involve cross-site working and community visits.
A full UK driving license and a vehicle for business use are essential for this post; however, reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.
