Job Responsibilities
* Coordinate and actively manage the Heart Failure service, ensuring accurate patient tracking and timely treatment within agreed pathways and targets.
* Manage referrals, book and reschedule clinics, monitor appointment outcomes, and escalate capacity or access issues as needed.
* Serve as a central point of contact for patients, clinicians and external teams, providing timely, professional communication and ensuring a patient‑focused approach.
* Handle telephone and other communications with sensitivity and confidentiality, assessing each situation and responding appropriately.
* Prioritise workload, book outpatient appointments via e‑referral/C&B, answer telephone queries, type correspondence and liaise with GP practices.
* Enter and extract data from Careflow and other Departmental and Trust systems, maintaining accurate records.
* Develop and maintain a team‑working philosophy, liaise with Specialist Nurses, and schedule appointments while managing clinical commitments within the Ambulatory Unit.
* Ensure systems support a high standard of communication, maintain professional relationships internally and externally, and organise own time effectively.
* Exercise judgement when dealing with enquiries, analyse and resolve issues, and work unsupervised.
Person Specification
* Good standard of education to GCSE or equivalent, including English.
* Proficient in Microsoft Office (Word, Excel, Access, PowerPoint) and Hospital‑based information systems.
* Experience in an administrative or secretarial role, with ability to work autonomously and prioritise workload.
* Knowledge of medical terminology and patient administration systems.
