Overview
Looking for a new challenge within cardiology? An exciting opportunity for a Staff Nurse on secondment/fixed‑term contract (up to one year, 22.5 hours per week) has arisen within Cardiac Rehab at Southend Hospital.
Responsibilities
* Assess and manage patients with Coronary Heart Disease and other cardiac conditions referred for Cardiac Rehabilitation, providing an individualised health‑education programme after hospital discharge.
* Contribute to the development and progression of the CR service.
* Adhere to the NMC Code of Practice and ensure compliance with hospital policies and procedures.
* Assess and provide health education and support for patients referred to CR from Southend Hospital, Essex CTC and other sources.
* Review ECG and test results and act appropriately; refer patients to other multidisciplinary team members as required.
* Participate in assessment and core rehabilitation, either by telephone or face‑to‑face.
* Risk‑stratify and assess patient suitability for inclusion on the programme and maintain regular contact while on the programme, using telephone, virtual or face‑to‑face methods.
* Liaise with ward and departmental staff, patients and relatives concerning specific patient needs, offering psychological support and appropriate resources.
* Liaise with other multidisciplinary team members such as CNS, cardiologist, dietician, counsellor and others.
* Promote continuous health education for cardiac patients in the hospital setting and post‑discharge.
* Educate Cardiology Ward Staff and implement the health‑education programme for patients with acute coronary syndrome and their carers.
* Review and help develop relevant health‑education literature as needed.
* Liaise with the ‘Take Heart’ cardiac support group and signpost patients to the self‑help group when appropriate.
