You, as a post holder, will work collaboratively with all secondary mental health services, including Primary Care Mental Health Link Practitioners, primary care services, and the Voluntary and Private Sector, to contribute to providing a Recovery-focused model of care.

The post holder will manage a caseload and work within the principles of the agreed model of care for the service as outlined in the operational policy. The role involves working as an autonomous practitioner within an integrated multidisciplinary team, including assessment, planning, implementation, and evaluation of care. The post holder will liaise with all involved agencies, be confident and competent in administering IM injections, and be fully aware of Medicines Management Guidance and related policies.
Person Specification
Education/Qualifications
* Qualified Mental Health Nurse registered with NMC
* Evidence of continuous professional development
* Educated to Degree Level
Experience
* Post-qualification experience within mental health services
* Experience working within a multidisciplinary environment/community
* Experience in NHS/Local Authority settings, understanding policies and practices
Knowledge/Skills
* Thorough understanding of mental disorders and treatment options
* Knowledge of the Mental Health Act 1983 and amendments
* Knowledge of NHS & Community Care Act, The Care Act, Mental Capacity Act, and related legislation
* Ability to assess and manage complex, high-risk situations effectively
* Good report writing and documentation skills, including legal and social reports
* Ability to coordinate professionals and services, and develop care plans in partnership with service users and carers
* Assertiveness, engagement skills, and ability to represent professional views
* Decision-making, problem-solving, and flexible approaches to meet service users' needs
* Excellent organizational skills, ability to prioritize and delegate
* Effective communication skills for diverse audiences
* Knowledge of CPA, NSF standards, and mental health practice skills
* IT skills and experience with service user data systems
