Job summary
* The post holder will work across a range of specialty Acute Medicine functions, primarily in Acute Hospital at Home (AHAH) and Outpatient Parenteral Antimicrobial Therapy (OPAT), but will also include Triage, Same Day Emergency Care, and AHAH northern services.
* Within the AHAH role, they will lead in delivering seamless transition between the acute trust and the community, ensuring patient safety and high-quality care. Current patient streams include OPAT, AF, COVID, AKI, Heart Failure, NSTEMI, Syncope, Palliative, and Frailty, with plans to introduce new streams.
* The post holder will proactively recruit new patients from AMU, SDEC, and wards, assess these patients, and facilitate their safe discharge into AHAH.
* They will work to improve and maintain the AHAH service based on ongoing needs and feedback.

We have a fantastic opportunity for a Registered Nurse to join our integrated Virtual Ward team, bringing experience and knowledge to a responsive, multidisciplinary team.
The Acute Medicine Specialist Nurse role addresses the growing needs of the local community, working across areas including Triage, Cardiac assessment, delivering Outpatient Parenteral Antibiotic Therapies, and Home and Ambulatory Care.
Virtual Wards support patients who would otherwise be in hospital, providing care at home to prevent avoidable admissions and support early discharge. The work involves remote patient care using devices feeding results into Epic, telephone assessments, face-to-face reviews, and ward visits to recruit patients and support flow.
This role offers an opportunity to embed digital technology in care delivery, aligning with national priorities to improve patient outcomes and system efficiency. You will also contribute to quality and service improvements, working closely with the Clinical Nurse Manager and multidisciplinary team.

Pay: The salary reflects the 2025/6 NHS pay rates, including a 3.6% pay award, which will be backdated if joining before implementation. Until then, pay is based on 2024/5 scales. For details, see NHS Employers.
