The Vale of Glamorgan Community Mental Health Service for Older People is a multi-disciplinary team of mental health professionals including social workers, psychiatric nurses and psychiatrists.
The team offers specialist secondary mental health care for adults living with dementia and people over 65years with mental health problems.
The team works alongside specialist NHS services to support people to live well and safely in their own communities through the provision of recovery focused care and treatment.

Main Place of Work: Llandough Hospital
To undertake UA reviews, provide information and support to social workers with assessments and reviews and monitoring of user and carers needs
About you
· Experience of working in a Team;
· Experience of working with people in a paid, caring role or voluntary capacity
· Some knowledge/ understanding of ageing/illness/disability and its effects of individuals/families and their Carers
· Good standard of written and verbal communication skills. Ability to organise and prioritise tasks. Negotiation skills
· Ability to work on an inter-agency basis.
