Mental Health Liaison Practitioner

An exciting opportunity has arisen for a 22.5 hrs band 6 Mental Health Liaison Practitioner to join the Mental Health Liaison team based in Princess of Wales hospital.

The Mental Health Liaison Team provides a service for older adults admitted to the general hospital as well as the emergency departments. The liaison team covers the Bridgend area outreach service for care homes within the Bridgend Area. The team offers timely comprehensive assessment of the mental health needs of those referred that present with unexplained medical conditions that may be a consequence of mental health illness or co-occurring mental health and physical health problems, and individuals presenting with incidents of self-harm.

The successful candidate will possess excellent communication skills, be a good team player with a flexible attitude and the ability to provide high quality individualised assessments and advice on the care and treatment of patients within the general hospital setting. They will work effectively within a multi-disciplinary team and demonstrate a genuine interest and enthusiasm for caring for people with mental health difficulties.

Main duties of the job

The main purpose of this role is to contribute to a 5 day a week provision of a Mental Health Liaison Service. May have to cover out of hours for the older persons community service on a rota system.

You will need to demonstrate a sound clinical knowledge and awareness of current clinical practice and have sound knowledge of safeguarding procedures, the Mental Health Act, Mental Capacity Act and the Mental Health Measures.

The post holder must have good communication, listening and organisational skills which are essential for the role along with excellent clinical assessment and formulation skills to meet the needs of the clinical team.

The post holder must have a positive attitude, be able to work as a member of a team and autonomously in a challenging and changeable environment and have excellent presentation, oral and written communication skills always acting in a professional manner.

Provide training and education for both Pre Registration and PostRegistration nursing students and other health care professionals/agencies.

Contribute to the development of a comprehensive liaison service.

Foster robust working relationships with colleagues from all disciplines within the general hospital setting and within the mental health services to facilitate improved joint working.

This post is fixed term/secondment for 6 months due to meet the needs of the service.

Welsh Skills Desirable: This post is advertised as Welsh Desirable. This doesn\'t mean essential; whilst the candidate doesn\'t need to have skills in Welsh, we\'ll consider it an advantage when short-listing and selecting candidates. This isn\'t fluency, just Speaking & Listening skills at Level 3 (equivalent to CEFR B2) or above. Level 3 means basic conversations with patients about their everyday health. Person Specification


Qualifications/Training

* Registered RMN
* First level professional qualification in mental health/learning disability
* Excellent clinical knowledge of specialist area in particular older persons mental health care


Experience

* Evidence of clinical practice in area
* Experience of liaison work
* Experience of inpatient nursing within OPMH
* Experience of working within a multi-agency community mental health setting


Skills/Knowledge

* Sound working knowledge of the legal and policy framework underpinning mental health service provision
* Knowledge of safeguarding procedures
* Clinical assessment and formulation skills
* Welsh language speaking/listening skills at Level 3 or above
