Job Summary
An exciting opportunity to join the West Community Services Team as a Clinical Care Coordinator for the Caernarfon / Dolgellau / Amlwch / Benllech CRT on a fixed‑term basis (until 31.03.2027). The successful candidate will support the CRT through referral triage, co‑ordinate rapid response, and discharge follow‑up, aiming to provide the right care at the right time and place. The role is integral to the Multi‑Disciplinary Team and requires close collaboration with all staff to meet targets and maintain a high‑quality, safe service for patients, relatives and staff. Excellent communication, organisational, pro‑active and leadership skills are essential.
Main Duties of the Job
* Manage a defined caseload in line with good practice, with responsibility over a 24‑hour period.
* Conduct complex needs assessment including CHC, health and social care, and unwell patients.
* Work collaboratively with other agencies to deliver patient services without direct supervision.
* Undertake a range of management duties and deputise for the Community Resource Team Leader.
Qualifications & Person Specification
* Essential
* NMC registered nurse.
* Experience working with patients who have acute or complex needs.
* Ability to work collaboratively with members of the Community Resource Team and other agencies within the wider health economy.
* Desirable
* Ability to speak Welsh.
